Syphilis is often hard to diagnos as symptoms can come and go. The first stage is usually marked by a single, painless sore that eventually goes away. In the second stage, a reddish rash will appear on one's hands and feet. With such indescriminate symptoms, it's important to get tested regularly especially if you are in a high risk group (homosexual men).

I think syphilis has taken a backseat to a lot of other STDs, This used to be considered a major STD, but now it's HIV, herpes, chlamydia, and so on. Like you mention the symptoms go away, which can make people think they're ok when they really aren't.
